1) Download the updated request form – https://educ.upd.edu.ph/wp-content/uploads/2023/10/Request-Form-8_24_2023v.pdf
2) Submit the accomplished request form at Room 104, Office of the College Secretary (or at educrequests.updiliman@up.edu.ph – via online requests) and secure a Statement of Accounts (SOA). Make sure to request a SOA before processing the payment.
3) Submit SOA at the Cash Office and pay the fee.
